English Birth Certificate Issuance
We only issue birth certificates for Taiwanese citizens based on the provided Taiwan government-issued documents, not authenticate the original birth certificate issued by the authorities from Taiwan, such as the hospital.
Since the household registration transcript (English version) contains both the English names of the applicant's parents and the relationship, it is a highly recommended document to be submitted as supporting evidence when applying for the authentication.

Please submit the following materials when applying:
For either walk-in applicants or applications via mailing:
- Application Form for Authentication: Each applicant must complete and sign the form. Applicants can submit multiple documents under one application.
- A clear copy of the Taiwan passport: Color/Black and white.
- Proof of the English names for the applicant’s parents: A document issued or certified by the Taiwan government, such as a clear copy of the parents’ Taiwan passport or household registration transcript (English version), as proof of their English name. Color/Black and white.
- Proof of the relationship between the applicant and parents: A document issued or certified by the Taiwan government, such as a clear copy of the applicant’s Taiwan ID card or household registration transcript, as proof of the relationship. Color/Black and white.
