- Required documents:
*Online filled visa form with signature
*Visa request letter stating the travel purpose and the tentative duration.
*India's medical organization or hospital's proof letter for the concerned diseases and necessity of medical treatment in Taiwan. The proof should be issued formally by the registered hospital.
*The medical visa is issued on the ground that the disease is categorized on the list [外籍人士及大陸地區人民得於臺灣地區接受醫療服務之疾病] of Taiwan’s Ministry of Health and Welfare.Please download the list
* Taiwan's Hospital's Medical treatment arrangement proof stating clearly the treatment necessity and procedure.
*The payment agreement between Taiwan's hospital and the patient, and the financial proof of the patient accordingly.
*Confirmed round trip ticket and confirmed hotel booking record
*2 passport size photos 3.5x4.5cm, white background, showing 60% face
*Passport original and photocopyIf the applicant is either the parent, spouse or children of the patient, the relationship proof (e.g. passport copy or birth certificate or marriage certificate) should be provided.
Submission:
Monday-Friday, 9:00-11:00.
Collection:
Monday-Friday, 15:30-16:30
There is no submission and collection other than the timings mentioned.
