3. Authentication for Vaccination Record, Health Certificate, or Medical Document
※ Our consular service jurisdiction covers four states, namely New York, New Jersey, Connecticut and Pennsylvania. We only accept documents produced within our service area. For documents executed out of our service area, please check the web link (Consular District) to find a competent mission.
※There is no designated medical institution for health check-up. You may consult with your primary care physician or any medical institution in our consular jurisdiction to do a health check-up.
If the application is submitted by an agent, please present a letter of authorization with the original and a copy of the agent's ID.
The Document to be authenticated has to be submitted or mailed in an envelope sealed by the medical institution (or physician); otherwise it has to be notarized by a notary public.

Chinese Translation
As the Health Certificate is in bilingual format, authentication of Chinese Translation is not needed.
As for a Vaccination Record or medical document, please note that a Chinese translation and the English original are to be authenticated as two documents. Chinese Translation has to be written with traditional Chinese characters.
How to have a Chinese translation authenticated:
(A) The applicant signs on the Chinese translation in person at our consular service counter or,
(B) if by mail, Chinese translation should be signed and notarized by a Notary Public; or
(B) With the English original authenticated by TECO, a Chinese translation can be notarized by a notary public in Taiwan.
Processing time, rate and payment
Regular: 10 working days at $15 per item.
Expedite (if applicable): 5 working days at an extra fee $7.5 per item.
Chinese translation at $15 per item.
Payment by cash, money order or cashier’s check (payable to TECO in NY). We don’t accept credit card or personal check.
How to submit an application
By appointment: Our consular service is available from 09:00 to 11:00, Monday through Thursday. Click to make an appointment online.
Walk-in: There is a limit of 25 walk-ins daily. We will start to handle walk-in customers from around 11:00 to 13:00 after those by appointment have been served. Walk-ins can take a number from 08:45 on a first-come-first-served basis.
Drop-off: An application placed in an envelope can be dropped off in our mail box at the ground floor lobby during our office hours.(Monday-Friday, 9 am - 5 pm)

To claim your document
by pickup: With the receipt or ID and no appointment necessary, picking up on the cited date or afterwards at any following Tuesday and Fridayfrom 15:00 to 16:00; otherwise by online appointment or walk-in-take-a-number during office hours. (Monday-Friday, 9AM-5PM)
By mail: Prepare a self-addressed-stamp-affixed return envelope. By doing so, the applicant agrees to take the risk of lost or damage during delivery, TECO-NY will not be liable for any accident.
By courier: Please provide a shipping label printout or email PDF file to consular.ny@mofa.gov.tw
