Power of Attorney (including Disclaimer of Right to Inheritance) Authentication
The document must be signed in the presence of our officer or notarized by a public notary from one of the six states under our jurisdiction. Please confirm with the relevant authorities that the format and content meet their requirements before applying for authentication. If there are attachments, ensure that clear copies are prepared and the complete documents are submitted in the correct order of pages before signing in front of our officer or handing to the public notary.

Please submit the following materials when applying:
For either walk-in applicants or applications via mailing:
- Application Form for Authentication: Each applicant must complete and sign the form. Applicants can submit multiple documents under one application.
- A clear copy of the applicant’s passport or ID: The passport holder must sign on the signature page.
- The Original Power of Attorney (including Disclaimer of Right to Inheritance):
- The applicant’s information on the passport must match the information filled out on the document.
- Must be signed before our officer, or have the public notary from the six states under our jurisdiction witness and notarize the signature.
- The duration of authorization cannot be earlier than the date of submission.
Notes:
- The document must be filled out ONLY using the information listed on the passport or ID, please confirm with the authorities if it meets the needs before applying for authentication.
- If a power of attorney has any attachment files, please have them ready before submission.
- Additional identification might be requested while the documents are reviewing.
