The certificate must be issued by the state health department under our jurisdictions. It must be in one of the following formats:
A digital file is applied and sent from the state health department’s website. Please print it out.
An original foldable card for immunization status records issued by the state health department.

For either walk-in applicants or applications via mailing:
Application Form for Authentication: Each applicant must complete and sign the form. Applicants can submit multiple documents under one application. If the person born is a minor, one of the parents should be the applicant.
Clear copies of the passports or state-issued IDs for the person born and applicant: The person born’s English name on the identification MUST match the English name on the Immunization Status Certificate.
Immunization status certificate: The certificate must be in one of the formats as described.
